In order to help you complete the application form as quickly as possible, it would helpful if you had the following information to hand. You will need to scan the below documents, as applicable for you, with this online application in order to be able to fully submit it.

  • Passport
  • Driving licence (both paper counterpart, and card) to include Cat C1 and less than 6 penalty points
  • National Insurance card (or subsequent proof of NI number)
  • Digital passport size photograph
  • All qualification certificates (HPC Paramedic, IHCD Ambulance Technician, university degree / diploma, etc.)
  • Current enhanced CRB (within 12 months for frontline 999 and 3 years for others)
  • Current registration card / certificate (HPC, GMC, NMC, etc.)
  • Annual refresher certificates (e.g. manual handling, vulnerable person, etc.)
  • IHCD blue light Ambulance Driving certificate (for frontline 999 is compulsory)
  • Referee details for two persons one of which must be current / most recent employer
